Festival Registration

One form per player. A parent or guardian must complete the indemnity and consent sections below.

Indemnity & Consent
As the parent/guardian of the above-named child, you:
  1. Grant permission for your child to participate in the Football Festival organised by the CAF-D Zwaanswyk Academy Class of July 2026 coaches.
  2. Acknowledge that football carries inherent risks of injury, despite every effort to ensure participant safety.
  3. Agree to indemnify and hold harmless the CAF-D Zwaanswyk Academy Class of July 2026 coaches, its officials, facilitators, hosts and volunteers against any claims, damages or liabilities arising from injury, loss or accident sustained during the event, except where such arises from gross negligence.
  4. Authorise the organisers to obtain medical treatment for your child in an emergency, and accept responsibility for any associated costs.
